The NTSB has finally pinpointed the main cause of this collision that killed over 600 people - erroneous maintenance. A week earlier, the 767 operating Flight 1638, had its engine replaced. However, the engine replacement was incorrectly done (sounds like AA191, doesn't?) and the major fuel line powering the engine was cut. The next week, on the first flight after the 767 was returned to service, the cut fuel line drained the tanks causing the aircraft to run out of fuel about a third of the way to Hong Kong. The 767 turned back towards Honolulu, however, they ran into a thick thunderstorm. In the clouds, neither aircraft's pilots could see each other, and the V2 777's TCAS warned the pilots too late. The 767's tail slammed into the 777's left engine and sliced through the fuselage ahead of the wings. The 777 split into two and spiraled into the waters below, killing all but one on board. The tail-less 767 flew on for 14 seconds, stalled, and crashed into the ocean, killing all onboard. Thanks to 707. #1638 #32 #Airlines #Central #Flight #V2
